How to Use the Plan Correctly
Step 1: Choose the Right Provider
- Search the First Health Network
- Call the provider’s billing office to confirm participation
- Use all preferred low cost ancillary services
Step 2: Receive Care
- Pay the provider (if required)
- Keep all itemized bills and receipts
Step 3: Submit or Review Claims
- Claims will be submitted by (In network) providers directly
- Fixed benefits are paid according to the plan schedule
- Payments ca go toward your medical bills or reimburse you
Step 4: Apply Benefits Strategically

How NOT to Use the Plan
